There are a lot of beep test videos on this site, though this one gives the clearest description I have seen of the test protocol and rules.

Large Group Performing Shuttle Run Test

Here is one guy doing the test solo from the start. If you get through to level 11, you can open part 2 which has the final levels. He makes it to about level 12. You could actually follow this online and try the test yourself.

Yo-Yo Endurance Test (part 2)

This is part two of the Yo-Yo endurance fitness test, the final few stages. This is the ‘endurance’ version of the yo-yo test, which is essentially the same as the standard Multistage Fitness Test (beep test). See also the posted part 1 of this video.

Soccer Beep Test

A group of soccer players performing the beep shuttle run test. Just a few of the shuttles during level 11 is shown, and none of them seem like they are ready to drop out. The course actually looks more like 15m rather than 20m. Notice the cones placed midway between the outer cones, which is a good way of keeping the players running straight up and down. Actually, you usually see soccer players performing the Yo-Yo test rather than the Beep test.
